New Student Health Forms and Immunization Requirements

Undergraduate Students

All undergraduate students are required to submit the forms listed in the new undergraduate student health requirements packet and enter individual immunization dates for required vaccines into the WPI Student Health Portal by JULY 1, 2025. 

Included in the new student health requirements packet are the following documents:

  • Immunization Record (individual immunization dates must be entered in the health portal)
  • Consent Form
  • Physical Examination Form
  • Tuberculosis (TB) Screening Questionnaire
  • Meningitis Waiver

Accessing the health portal

Log into our Secure Health Portal to upload forms and documents and access important health-related messages, laboratory and x-ray results, educational handouts, and more.

Please make sure to use your WPI username (not your full email address) when logging into the WPI Student Health Portal. 

How to add immunization dates

After you upload your forms into the online secure patient portal, please input each immunization date under the “immune” tab as documented on your health record. You must upload your health forms AND input the immunization dates for each required vaccine for the Health Services Staff to review.

Submitting health forms

Forms can be completed and uploaded to the Student Health Portal. We will only contact you if we require additional information. Please check your WPI emails - we will send you messages in the health portal, but notifications go to your WPI email address. ​Please do not email regarding health forms; it is not a secure way to transmit confidential health information.

How do I file a vaccine exemption?

WPI provides a process for students to submit a request for a medical or religious exemption for all required vaccinations. Students must complete a Vaccine Exemption Form and specify whether they are asking for a medical or religious exemption. If a student requests a medical exemption, they must upload a Medical Provider Exemption Letter from their healthcare provider in addition to the Vaccine Exemption Form. See the instructions below. 

To access and upload the exemption form, please complete the following steps: 

  1. Download, print, complete, and sign the Student Vaccination Exemption Form.
  2. To upload the completed form, log into the WPI Student Health Portal
  3. Select the upload tab; scroll down to see the drop-down list of documents available for upload.
  4. Select "Student Vaccine Exemption Form" and upload the completed exemption form.
  5. Select "Medical Provider Exemption Letter" from the drop-down list, select file, and upload the exemption letter if required. 
What is the best way for me to submit my required health forms?

The preferred method is electronic upload to our secure health portal. To log in, use your WPI username (not your full email address) and password. Remember, always keep hard copies of these documents for your records.

How do I fill out the tuberculosis (TB) risk questionnaire?

Please log into the WPI Student Health Portal. After logging in with your WPI credentials, please click on the "forms" tab on the left side menu. Complete the New Student TB Risk Assessment 2025". Once complete, you will be instructed on next steps.

If you are determine to be at high risk for tuberculosis based on the online form, you will need to reach out to your medical provider at home to conduct the necessary screening test(s). Your medical provider will need to complete the following form:

What is one of the required vaccines isn't offered in my country?

Ideally, we can suggest ways for you to get your vaccine prior to coming to campus, such as going to a travel clinic or urgent care clinic. Otherwise you will have to make arrangements to get your vaccine as soon as you arrive in the United States, however, this is for extenuating circumstances only. Student Health Services typically arranges vaccination clinics once/year, typically in October, when you can get many of your vaccines (please check with your insurance company regarding coverage of vaccines). Otherwise, it will be your responsibility to get all of your required vaccines. 

How do I waive or enroll in the WPI Student Health Insurance Plan?

All students enrolled at least 3/4 time are required by Massachusetts State law to enroll in the Student Health Insurance or be covered by a health insurance plan with comparable coverage. To ensure compliance with Massachusetts law, all students registered for 7 or more credit hours will be automatically billed for the Student Health Insurance Plan. Students must actively attend classes for at least the first 31 days from the start of classes to remain eligible for the WPI SHIP. Students enrolled in only online courses are not eligible for the plan. To read the details of this law, click here

Students must submit a waiver or enrollment form at www.universityhealthplans.com/WPI by the deadline of August 31, 2025

Please note, health insurance information sent to the Student Health Center or Athletic Office does not get conveyed to the Bursars Office, and therefore does not activate the waiver process.  In addition to, and separate from providing insurance information to the Student Health Center or Athletic Office students must submit the annual health insurance waiver form at www.universityhealthplans.com/WPI.

Will you accept my doctor’s office’s forms for physical exams and/or immunizations?

As long as the physical is within the last calendar year and signed (electronic or e-signature is fine), we will accept them. Immunization records also need to have a signature of authorized personnel. If you had titers drawn because you do not have documentation of vaccines or illnesses, please also submit these blood results.

What if my physical exam or immunization is scheduled for after the summer deadline?

Please make sure to upload documentation of the required forms and immunization documents as soon as possible if they are scheduled after the deadline. You will continue to receive regular communication from our office until your medical record is complete, and you are compliant with all of our required vaccines and health forms. 

If you have a physical exam from last year, you can upload that to the WPI Student Health Portal until you obtain the updated document.
